Licensed Clinical Social Worker - Stony Brook Psychiatric Associates, UFPC 

Location: Stony Brook, NY

Schedule: Full time

Days/Hours: Monday - Friday; 8:30 AM - 5 PM 

Salary Range: $54,071 - $85,520 (pay will be determined based on experience, preferred qualifications, and education)

Responsibilities

SUMMARY: The Licensed Clinical Social Worker must have strong organizational and communication skills (both verbal and written). The Licensed Clinical Social Worker functions as a member of an interdisciplinary team and provides clinical social work services to patients and their families to improve or maintain social, emotional, functional and physical health: provides consultation, education and training to staff regarding psychological issues that impact the delivery of optimal health care services; and participates in the development and delivery of education, research and continuous quality improvement. Demonstrates competencies defined in Clinical Social Worker responsibilities in providing age-appropriate care to patients from the age of birth to 21 years and some select adult patients. The successful incumbent must have excellent communication and interpersonal skills required for interaction with all stakeholders. The incumbent is expected to demonstrate a high level of professionalism, organization, implementation/evaluation of actions required to meet the patient’s health and human services needed.

 

Job Duties & Essential Functions:

  • Able to provide psychotherapeutic services utilizing various modalities and milieus.
  • Screen and triage all referrals.
  • Conduct comprehensive intake on patients with complex behavioral health needs and provide necessary counseling services and referrals.
  • Screening and identification of patients.
  • Establishment of an effective relationship with patient/family.
  • Assessment of patient/family strengths and needs.
  • Development of a family-centered plan of care.
  • Procurement and coordination of services.
  • Monitoring and evaluation of progress.
  • Facilitates the patient/family’s ability to assume responsibility for managing their own health care when appropriate.
  • Promoting return of the patient/family to the highest level of wellness possible.
  • Assisting in the acquisition or verification of financial/payer/insurance information when appropriate.
  • Provides relevant and timely psychosocial assessments of patients and families.  Assesses the child/family needs for case management. oversight/intervention.  Collaborates with appropriate care coordinators to facilitate continuity of care and optimal health outcomes.
  • Provides crisis intervention as necessary to increase patient/family functioning and facilitate health management.
  • Provides individual, group and family brief counseling to facilitate the resolution of identified psychosocial problems to increase patient/family functioning and facilitate health management.
  • Promotes multidisciplinary collaboration between all members of the health care team to facilitate and coordinate quality patient care in a timely, cost effective manner.
  • Understanding in providing a holistic approach to a patient’s behavioral health needs.
  • Perform all other duties as assigned by management.

Qualifications

Required Education & Qualifications:

  • Master’s degree from an accredited program.
  • Valid New York State Social Work License with LCSW Certification.
  • 3 years of experience providing behavioral health services to individuals, families and groups.
Preferred Qualifications:
  • Experience working in a fast-paced medical practice.
  • Experience with EMR.
  • Excellent communication and interpersonal skills required for interaction with all stakeholders.

Physical Demands:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to communicate with patients, staff and medical providers. The employee must be able to exchange accurate information in these situations. This position is active and requires the employee to constantly move about in the office to assist patients, staff and medical providers. Any additional physical demands will be outlined and provided by management.
